Programa Mais Médicos : limites e possibilidades para mudanças nos processos de trabalho e na produção do cuidado na estratégia saúde da família

Resumo
This study analyzes the effects of the insertion of a Cuban doctor of the Mais Médicos (More Doctors) Program (PMM) into the work processes of a Family Health Strategy (FHS) team, as well as discusses its limits and possibilities for the production of changes towards the comprehensiveness of health care. This is a qualitative, descriptive-exploratory research, of the case study type, carried out in a city of the state of Espírito Santo, Brazil, having as unit of analysis the team in which the referred doctor works. Also participating in the study were a nurse, a nursing technician and six community health agents. Empirical data production techniques were used: participant observation of the team's assistance activities; in-depth interviews with the Cuban physician and the nurse, and a focus group, in which the nursing technique and the community health agents participated. Data analysis was based on theories of the work process and intersubjective production of health care from the perspective of comprehensiveness. The results indicate contributions in the expansion, diversification and continuity of care actions, as well as in the strengthening of the links between team and users, especially in the doctor-user relationship. Among the challenges, the persistence of practices segmented by professional categories, subordinate to the (bio)medical figure and knowledge, with limited interprofessional and team-community interactions in the construction of common projects of care. It is hoped that this study will stimulate future debates and research on PMM in the field of Collective Health and contribute to the exchange of knowledge related to the work processes in Primary Health Care (PHC) and the Unified Health System (Sistema Único de Saúde - SUS), bringing them closer the needs of the Brazilian population and the workers themselves, and offering subsidies for the improvement of health care in Brazil.
