Incidência de infecção nosocomial causada por vírus respiratório, em uma unidade de cuidado intensivo e semi-intensivo neonatal

The newborn infant, especially one born before term, is more susceptible to infections. There are few published data on nosocomial viral infections in the neonatal period and those have conflicting results. Aims: to determine the incidence of nosocomial viral infection in newborn infants hospitalized in an intensive care unit (ICU) and semi-intensive care unit, at Vitória, ES, and to carry out clinical surveillance for the emergence of symptoms suggestive of that viral infection. Methods: this is a 13-month prospective cohort study held conducted from November 2010 to December 2011. All newborn infants hospitalized in the neonatal ICU of the Cassiano Antônio Morais University Hospital (HUCAM) throughout the study were subjected to clinical monitoring and weekly collection of nasopharyngeal secretion (NPS) up to patient discharge. Fifteen different viruses were searched through indirect immunofluorescence (RIFI) and polymerase chain reaction (PCR) in the multiplex format. Results: one hundred and fourteen newborn infants were included in the study, of which were obtained 424 samples of NPS. All samples were tested by IIFR and 51 of them by PCR. Twenty-six (22.8%) newborn infants showed clinical suspicion of nosocomial viral infection. In nine patients was detected some respiratory virus, being respiratory syncytial virus (RSV) in four (3.5%), Rhinoviruses (HRV) in three (2.6%) and Influenza A virus (FLU) in two (1.7%). Two patients were asymptomatic. The incidence of nosocomial viral infection was 7.8%. We found a weak association between the clinical suspicion of viral infection and detection of the virus in laboratory. Conclusions: the noted incidence was compatible with the characteristics of the studied unit and the result of an ongoing effort to prophylaxis and continuing education. Considering the difficulty of accurate clinical diagnosis in neonatal period, we emphasize the importance of laboratory diagnosis of these viruses in that period.
