Introduction: Venous ulcers present an important health issue, for an elevated number of people that suffer from this condition access the healthcare service seeking care for it. The assistance to the person with venous ulcer demands nurse’s planning, strategies, and interventions, with a holistic approach, and which produce an adequate registry. Objectives: To design and validate a Nursing Consultation Instrument for the Person with Venous Ulcer assisted at the Basic Care level, oriented by the Theory of Basic Human Needs of Wanda Aguiar Horta. Methodology: This is a methodological study developed in three stages. Stage 1: a) The identification of the basic human needs variables for the person with venous ulcer found in the integrative review of the literature; b) The design of a research instrument containing the selected variables for each need, disposed in 3 point Likert type scales distributed by 1=never relevant; 2=sometimes relevant, 3=always relevant. State 2: a) Validation by teaching nurses at the Federal University of Espirito Sando, integrated to the subjects of Adult Healthcare, Nursing in Adult Healthcare, Curricular Internship I, and Curricular Internship II, and by assisting nurses acting at the basic healthcare in the Municipality of Vitoria; Stage 3: The realization of the data collection instrument for nursing consultation of the person with venous ulcer containing the indicators validated by the nurses from the empirical variables with relevance level ≥ 0.80. Results: 96 empirical variables distributed in three domains: Identification of the Person, Interview, and Physical Exam; with 81 psycho-biological needs, 15 psycho-social needs, and zero psycho-spiritual needs, which were validated by the group of nurses judges. Product: A Data Collection Instrument for the Nursing Consultation applied to the person with venous ulcer. Conclusion: The data collection instrument in the nursing consultation is shown to be a potential tool for the organization of the nursing assistance in basic care, supporting the clinical analysis and in decision making. There is a perspective of the aggregation of the instrument to the nursing consultation of the person with venous ulcer to the activity of nurses acting in the basic care in the Municipality of Vitoria, and the further inclusion in the Rede Bem Estar program, in the same Municipality, for the use in the electronic healthcare records as a supporting tool for the nursing assistance.
