A luta antimanicomial e a política de saúde mental na voz dos militantes do movimento pela reforma psiquiátrica

dc.description.abstractThe goal of this study is to analyze the direction of mental health politic (MHP) (2001 – 2015) on the perspective of the militants from the psychiatric reform`s movement (PRM) by the light of the movement`s struggle flags. That thinking is built considering a scenario marked by the resistance from some segments to the closure of psychiatric beds, to the most recent expansion of religious therapeutic communities, to the neoinstitutionalization`s process of individuals in psychological distress and yet, to the changings in the management of the mental health`s national coordination from the health ministry. There is a hypothesis that in a context of competing projects, the psychiatric reform is losing some of the advances made since the 1990. Therefore, it was used the oral history as a method, with semi structured interview, done with nine PRM`s militants. For the study, we used this content analysis. The results show that the initial movement flag, of the reform of hospitals, was replaced by the flag of “a society without mental hospitals”, marking a trajectory of deinstitutionalization. The flag of the anti-mental hospitals keeps on the movement’s agenda, considering that the psychiatric hospitals still put themselves as part of the treatment after the approval of the law 10.216 in 2001. This fact shows that the mental health field is strongly tensioned by different agents that from their interest, they stand against as much as in favor of the psychiatric reform. It also marks the tension that these groups have on the state apparatus. The movement takes a critical position in relation to the implantation of MHP. Facing the conditions of mental health services, the militants claim for the strengthening of not only these kind of services, but public services in general. Although there is no consent among the militants, over the directions of the MHP, they condemn the neoinstitutionalization`s process, with new ways of institutionalization, by increasing and creating beds in therapeutics` communities. Therefore, between advances, setbacks and challenges, they emphasize that the struggle on the mental health field can not be detached from a larger social struggle field, being necessary, therefore, the strengthening of anti-mental hospitals` struggle in time of crisis.
dc.description.resumoO objetivo deste estudo é analisar os rumos da Política de Saúde Mental (PSM) (2001-2015) na perspectiva dos militantes do Movimento pela Reforma Psiquiátrica (MRP) à luz das bandeiras de luta do movimento. Esta reflexão se dá considerando um cenário hoje marcado por resistência de alguns segmentos ao fechamento dos leitos psiquiátricos, a expansão mais recente das comunidades terapêuticas religiosas, o processo de neoinstitucionalização dos sujeitos em sofrimento psíquico e ainda, as mudanças na gestão da coordenação nacional de saúde mental do Ministério da Saúde. Tem-se como hipótese que em um contexto de projetos em disputa, a reforma psiquiátrica vem perdendo alguns dos avanços conquistados desde a década de 1990. Para tanto, foi utilizada como método a história oral, com entrevista semiestruturada, feita a nove militantes do MRP. Para a análise, utilizamos a análise de conteúdo. Os resultados demonstram que a bandeira inicial do movimento, de reforma dos hospitais, foi substituída pela bandeira ‘por uma sociedade sem manicômios’, marcando uma trajetória de desinstitucionalização. A bandeira antimanicomial continua viva na agenda do movimento, considerando que os hospitais psiquiátricos ainda se colocam como parte integrante do tratamento após a aprovação da Lei 10.216 em 2001. Este fato evidencia que a área da saúde mental é fortemente tensionada por diferentes atores que, a partir de seus interesses, se colocam tanto contrários como a favor da reforma psiquiátrica. Marca ainda o tensionamento que estes grupos exercem sobre o aparato estatal. O movimento assume um posicionamento crítico em relação à implementação da PSM. Diante das condições dos serviços de saúde mental, os militantes defendem o fortalecimento não só destes, mas dos serviços públicos em geral. Apesar de não haver um consenso entre os militantes, no que diz respeito aos rumos da PSM, esses denunciam o processo de neoinstitucionalização, com novas formas de institucionalização, por meio do aumento e criação de leitos nas comunidades terapêuticas. Assim, entre avanços, recuos e desafios, enfatizam que a luta no campo da saúde mental não pode ser descolada de um campo maior de luta social, sendo necessário, portanto, o fortalecimento da luta antimanicomial em tempos de crise.
