Resumo
The implementation of the Family Health Strategy (ESF) as a mean to improve primary health care and redirect the model of health assistance hasn’t been able to change the historical predilection of Vitoria’s Single Health System (SUS) users for emergency health services. Identifying critical reasons that lead users to seek assistance at the Emergency Unit (PA) of a territory completely covered by ESF is the main objective of this study. The present study focuses on user’s opinions on health services based exclusively on their previous experiences. Obtaining an extensive understanding on the organization of these services requires comprehending the point of view of those who use it. A qualitative approach was used for the development of the study. Information acquired through semi-structured interviews was tabulated using as strategy the Discourse of the Collective Subject. Its analysis was based on the Content Analysis method proposed by Bardin (1979). Results indicate deficiencies on the organization of Family Health Units (USF) due to poor accessibility and lack of resolution to user’s health issues. Functional accessibility is the most compromised because of insufficient resources and health professionals, mainly doctors. Resoluteness is associated to minor availability and low agility of health care. These factors are the central reasons for seeking at Emergency Units (PA) complementary treatments to the ones already initiated on the USFs and also for non-urgent situations, even thought it is known by population that Emergency Units (PA) are a reference service for health emergencies only. Another interesting result extracted from the interviews regards the operational suggestions given by users to improve the assistance and care provided by USFs. These suggestions should be embraced by the USFs so that they become able to respond with quality care and assistance to the demands taken by users to other health services. Into the perspective of a health system focused on users, aiming for integral care through preventive and health promoting as well as curative actions and carried on by multidisciplinary knowledge and practical changes on work process in health, this study shows the need to guarantee user’s access to health system through its various access points, the capacity of health system to embrace and respond to user’s demands and afterwards lead them, in an appropriate and opportune way, to the specific health territory responsible for managing long term assistance.
