- ItemA genealogia de um mestrado em saúde coletiva(Universidade Federal do Espírito Santo, 2007-07-30) Silva, Roberta Scaramussa; Andrade, Ângela Nobre de; Rosa, Edinete Maria; Szymanski, HeloisaIn Brazil, the Public Health policies were, until half of the 20th century, characterized by clientlism practices, based on a model of sanitary campaigns, in which it was adopted a repressive disciplinary style of medical intervention on the bodies, both individual and social. After 1960 is noted an intense popular movement searching for changes in different social areas to implement actions resisting the instituted powers. Among those, the attempts for effective transformations in the Health area, in favor of a reform that would overpower the biologic, curative and assistencialist model that existed, having its peak on the decade of 1980, with the Sanitary Reform. The recent movement, called Colective Health appears in this context to breach the speech- practical supported by the hegemonic model of Public Health. This field of theory and practice is committed with the entry of Social Sciences in the Health Science, with the widening of the health concept and with the autonomy of the subjects involved in the process, rescuing the meanings of politics and public lost so long ago. Based on that, was the objective to develop a genealogic evaluation of the Program of Master degree in Colective Health of UFES, based on the speech of its students that were currently working in the public health services. Were interviewed eight students. The interviews were taped, transcribed and discussed based on the Speech Analysis. The speech was organized according to three more recurrent themes: the undergraduate formation, the professional practice on the public service and the formation on the Master’s Program. The genealogy of the speech produced by the students showed a permanent shock of forces in which the students stated the processuality (ethical meaning) or, in the contrary, the denial of it through the imposition of the knowledge (moral meaning). In a general way, in the field of the production of knowledge, either in the undergraduate formation or in that Master program, there was the predominance of heteronomous practices (moral). The incorporation of the knowledge to the logic of the market leads to the making of workers that follow the demands of the capitalism, increasing the non commitment with a critic and inventive learning. The speech evidences the separation between the academic contents and the practice on the public health services, reveling itself in dissatisfaction with the practice and the belief that the permanent formation might lead to the answers of the challenges imposed by the everyday job. Referring to the professional practice, the collective production does not happen, the knowledge of the other (either the coworker or the client of the health system) is constantly denied or ignored. The multiprofessional practice exists much more as something imposed by the necessity of coping with the complexity among the fragmentation of the knowledge. Even with that, thought less evident, in some moments it’s possible to see some attempts of breaking through with the instituted forms. Even thought it does not appear as an ethical ways of being, it opens an important place to the emerging of new ways to think and make health.
- ItemAos cuidadores do serviço residencial terapêutico: novas maneiras de cuidar...?(Universidade Federal do Espírito Santo, 2009-04-08) Ribeiro Neto, Pedro Machado; Avellar, Luziane Zacché; Andrade, Ângela Nobre de; Moreira, Maria Inês BadaróThis work has proposed conducting an approximation of the reality of caretaker of a SRT to contribute to discussions on the Psychiatric Reform and deinstitutionalization of madness. We chose the qualitative approach because we believe that best apply to the proposed objectives. The study participants were six caretakers of a therapeutic residence of the State of Espírito Santo. For data collection was used semi-structured interviews and participant observation. For data analysis and interpretation were used the "Content Analysis" of the material collected, and prioritized the use of one of its categories, the "thematic analysis”. We found that caregivers are professionals who worked on common services basically, without a higher level of complexity, and also unrelated to the area of Health. All participants have less than a year of experience in the SRT. Were contracted of two companies: one specializing in food, and one of general services. Participants considered that were not prepared to act in the house, but despite this, they feel prepared for the service. Had no training in health, acquired the main information of the SRT on their first contact the HAB, which received guidance on their work in residential service. Besides the guidelines, learn to deal with situations, which were mainly concerned in the day to day work. Was noted that users, in general, are similar to the child several times in the speech of the participants, and the process of social reintegration is compared to the child learning. We suggest the creation of spaces where can be discussed with the caregivers and residents on issues such as health care, madness, and that the professional of residential services must have a considerably more comprehensive support for the conduct of their office. In this context, beyond inquire about which types of care are being practiced by caretakers of residential service for residents, we concluded that also indispensable discuss the level of attention given to caregivers.
- Item"Corpos marcados e fé na vida ...": mastectomia e políticas públicas de saúde da mulher(Universidade Federal do Espírito Santo, 2007-08-10) Sousa, Aline Lopes de; Andrade, Ângela Nobre de; Avellar, Luziane Zacché; Portugal, Maria Amélia LobatoThe population has always been away from the discussion involving public health policies in Brazil. Most of the time it reflects a mere technical relationship between doctor and patient, as well as public health services and the population. In the 80 s and 90 s decade there is a reengineering of the system and SUS is implemented. Prevention and cure are integrated and the population is also heard. Considering how lethal breast cancer can be, it is essential to keep the population participative. The purpose of this study was to explore the consequences of breast cancer in women s life, their demands, as well as their relationship with the public health policies. Eight women who had had breast cancer and had the assistance of SUS during the treatment in Vitória, ES, were interviewed. All of them had the diagnosis about three years ago. These women were able to share their experiences from the time they learned about their illness, to the surgery and treatment. The data was collected using the Oral Story technique, taking into account the importance of the context lived by those women throughout their life, especially their experience with breast cancer, focusing on mastectomy. The Content Analysis was used to study this data. It was possible to observe that, before the diagnosis, these women thought about cancer as immediate and certain death , which consisted in great suffering. After the surgery they had to face the restriction of routine activities and the wish of breast reconstruction. Even so, they were able to fight for life quality. It is possible to see that, although the public health policies offer service to the women who have breast cancer, it takes them several months to get an appointment with the doctor after they find out about it. Besides, the professionals in Espírito Santo say they earn very little for the breast reconstruction, and this service was interrupted. It is important that everybody fights for their rights.
- ItemDinâmica internação/desinternação em processos que resultaram em internação compulsória em hospital psiquiátrico(Universidade Federal do Espírito Santo, 2009-08-31) Pesente, Lucinéia; Andrade, Ângela Nobre de; Cupertino, Christina Menna Barreto; Souza, Lídio deThe citizenship of mental disorder (PTM) carriers has been claimed by a psychiatric reform movement. This reform pretends to question and also to extinguish the traditional psychiatric care model, which is self centered at the hospital institution. In this context a reduced hospitalization number triggers the use of a different strategy to supply this demand: hospitalization by the use of a law enforcement decision. In this particular case the hospitalization and discharging decisions are not entirely determined by the medical and care professionals but by a judicial decision. During the first semester of 2008 these ordered hospitalizations occupied, 11 of the 50 available berths for patients under crisis events, at the Short Permanence Unit (UCP) of Psychiatric Hospital (PH). Objecting to better understand this reality, this research analyses the prosecutions which resulted in hospitalizations and discharges at PH during 2008 first semester. The present work is a documental research with a qualitative approach and its sources were the judicial prosecutions that ended up in law enforced hospitalizations. Among the eleven identified cases, six could be analyzed by its prosecutions. The analyzed material were copied directly from the lawsuits or transcribed by the recorded readings performed during the constant visits to where they were stored. The data were analyzed by the content analysis proposed by Bardin(1997). The three principal axles of the analysis were developed over the hospitalizing/discharging dynamics: the family, the justice and the hospital. The main themes were distributed within these principal axles once they were interconnected. The data indicate that the time of enforced by law hospitalization 28 times greater than the non enforced. Beyond that, the responsible claimants of five hospitalizations had low incomes and lived under the pressure of other relatives who also required special care. The sixth case is of an individual who lived since childhood at institutions, and the last one of them were the claimer of the hospitalization. All the hospitalization claims allege PTM's agressivety. All the enforced claims initiated by an interdiction request, and all of them were attempted without a real investigation; the hospitalizations however presented custodial character. The hospital presents itself to be impotent against the law enforcements, ever since it can not deny them under the risk of the law penalties consequences; but also appears active in this process, constantly reporting to the justice about the conditions of the PTM's related to their discharges. We could notice that the hospitalization take different meanings to the evolved parts in its dynamics: to the family, it is a manner to be relieved from the overload they feel incapable to bare; for the justice, it seems to represent the prevention of a possible aggressive act, once there is already a history of danger; and for the hospital it is extreme measure that deviates its psycho stabilization basic purposes, furthermore it represents a throwback of the psychiatric reform principles.

- ItemHomens e saúde: discutindo interações no serviço da atenção primária(Universidade Federal do Espírito Santo, 2009-11-16) Mendonça, Vítor Silva; Andrade, Ângela Nobre de; Morato, Henriette Tognetti Penha; Trindade, Zeidi AraujoThe man's health as question appears to be approached and studied from the behavior of risk adopted by the own subjects about sex male, many often rooted in the dictates of a hegemonic masculinity socially imposed. This behavior depicts certain absence of responsibility of men in relation to health care do not admit a state of weakness or fragility caused by disease. In Brazil the issue is no less serious well studies show that man does not have prevention habits and more the men themselves are placed in the margins of public health policits, aimed primarily for women. From then on is regarded as fundamental to think the inclusion of men in the decisions regarding health because the rates of male mortality and morbidity have been gaining importance on the national scene due to the incidence of malignant neoplasms and traffic accidents, mainly. This study aimed to investigate the conceptions of men users of a health service in primary care, the city of Vitória, about health and service offered to the population. It utilizes as a reference the production of Georges Canguilhem (1904-1995) that in general considers health for its normative plasticity i.e. the individual has the ability to incorporate their own rules to new situations without losing the capacity for action. The study included 35 users of the health service with aged between 25 and 54 years old. The information were collected through open interviews which allowed access the questions about the reality in regarding the health practice of that men and were evaluated by content analysis. It was observed among other things that most men still have their health actions aimed only to times of extreme need or they only seek the health service when their situation interferes with something of greater importance as work. Notes that 94.2% of participants have a preference and feel included when the health service is offered in time is compatible with the work. However, men are not seen included in programs or activities of the health units determined by the Ministry of Health as the gateway to the Unique Health System. In addition, notes also that 22.8% of that male population yet have the initiative to seek the service as a means of promotion and prevention of it health and may signal the institution of other existential modes in relation to the hegemonic model of masculinity once it seems to appear a new culture of men more worried about their health. However, we must remember that is a process slow gradual and demand time to be effective. It is hoped that those results may to generate subsist for the reflection about the public health politics aimed at the man.
- ItemNa corda bamba: reforma psiquiátrica e saúde mental na atenção primária(Universidade Federal do Espírito Santo, 2004-08-23) Romanholi, Andréa Campos; Andrade, Ângela Nobre de; Trindade, Zeide Araújo; Amarante, Paulo Duarte deThis study discusses about the limits and possibilities of the Brazilian psychiatric reform implementation in the primary health care. Services on this issue have been assumed as a doorway to the Brazilian health system (SUS), however, they are not understood as part of the net of this system when it is concerning to people affected by madness. Trying to investigate that fact, this study has been worked with the primary health care professionals, through application of semi-structured interviews, questionnaires and focus group; so that, it was possible to get to know the difficulties and possibilities in order to transform that reality, and count on this service during the reform process. It could be assumed that, in the professionals point of view, the Basic Health Units have an important role in the psychiatric reform, once they are effectively territorial services, which allows one to get to know the users and their reality; and to accompany them throughout the several moments of their lives. However, those professionals reported several difficulties that make their possibilities to abandon the traditional practices of health extremely tough. They are very affected by the reality they face, by the excessive demand, and by the work process that is still bureaucratic and incoherent with the transformation proposals. Those difficulties refer to the attention to the general problems of the health field, and also interferes in the possibility to assume the attention of the people with mental problems that are seen as people who are required larger flexibility from the services to be attended. However, regardless the need of larger know-how, the mentioned difficulties are the same ones faced for the construction of a new model of attention to the health and the relationship with the mental health area is received in a positive way. About it, there are some expectations that this mentioned area, with its know-how and techniques, helps them to overcome difficulties. On the other hand, the work in the primary health care has shown a more complex reality than that which has being worked on the field of the reform. The last one deals with users who are victims of violence and exclusion that which is typical in the traditional psychiatric thought and in the primary health care there are those and more the enormous amount of people that suffer the violence of the social and economical exclusion, by living in poverty with lack of life perspectives and threatened by the informal but influential nets of the “local power” (coming from drug traffic and family violence). So, it is evaluated that the union of those two fields (psychiatry reformation and primary heath care) may enrich both, so that, it will cause the transformation of their practices and the enlargement of the invention opportunities.
- ItemPromoção da saúde: uma nova política pode alterar um paradigma?(Universidade Federal do Espírito Santo, 2008-08-26) Pinto, Elzimar Evangelista Peixoto; Andrade, Ângela Nobre de; Morato, Henriette Tognetti Penha; Trindade, Zeidi AraújoThe research analyses the National Politics of Health Promotion, developed in the ambit of the Sistema Único de Saúde- SUS (Unique System of Health), aiming at analyzing the conceptions of health that permeates the politics. It analyses its approximation with the several chains of Health promotion and the possible contradictions between the goals and strategies of consolidation. It is a documental research, done through a primary document – the National Politics of Health Promotion text - and uses as method the methodology of content analysis. It evaluates that the referred politics keeps the traditional emphasis in the orientation for the change in behaviors, highlighting the individual aspect, in a vertical and authoritarian way. Such perspective, when considering the subject as responsible, contributes for the non-politicizing the concept of health. The strategies to face the social determination of health are vague and imprecise. The evaluation of actions of promoting health is advised through the technical knowledge, not considering the participation of all the actors involved in the process. It emphasizes the importance of formatting health professionals to work with the health promotion, seeing such formation as skill for the counseling of healthier life styles. Such proposals show a contradiction with the directives of SUS, which stand out the importance of autonomy and the popular participation in the health production. To conclude, it is important to stand out the necessity of orientations reflections and practical actions, proposed by the National Politics of Health Promotion, searching for the production of new relations between humans, related to a potentiality of individual and collective practices.
- ItemRevelando o acolhimento familiar: um estudo sobre a experiência de acolhimento para a família acolhedora(Universidade Federal do Espírito Santo, 2012-08-16) Lima, Nayra Erlene; Andrade, Ângela Nobre de; Nascimento, Célia Regina Rangel; Guareschi, Neuza Maria de FatimaFoster care is a type of care intended for temporary care for children and adolescents who are living away from their family. This study’s aim is to investigate and analyze the relationships built between foster family and children. Therefore, we conducted semi-structured interviews in three host families, in addition to the social worker and the psychologist responsible for monitoring the program's actions. The data was submitted to Bardin content analysis method. The results showed that the participants considered the experience a positive one, usually associated with the aspect of giving aid to helpless children. The study also revealed that for the foster family, the fostering experience can also have a positive aspect as in living together as a family and their efforts in providing the care that it believes is necessary, referencing the socially widespread models of maternity and paternity. The fostering experience is also noteworthy because it presents possibilities for building relations of affection and reference for those who are taken in, although inserted in a dimension of temporariness. The studies aimed to deepen into this theme are of great importance in order to contribute to the theoretical study on care practices for children and adolescents. Moreover, it is believed that such studies could support the development of more effective public policies that consider the particularities and the plurality of the public attended, becoming thus a further guarantee instrument and promotion of human rights.
