- ItemA assistência farmacêutica e o seu processo de reorientação no município de Vitória/ES : avanços e desafios(Universidade Federal do Espírito Santo, 2013-03-26) Simões, Fabiane Lima; Figueiredo, Túlio Alberto Martins de; 1º membro da bancaVitória, Capital of Espírito Santo State, is a reference to other municipalities regarding the process of reorientation of Pharmaceutical Assistance. It is concerning this object - the Pharmaceutical Assistance and its local reorientation – that this study was conducted with municipal administrators and Pharmaceutical Assistance managers between 1998 and 2012. Thus, the aim of this study, which included the participation of three administrators and three managers, was to analyze on a Socio-Historical perspective, the institutionalization of Pharmaceutical Assistance in the county and its subsequent reorientation process. As devices of material production it was used documental observation, interviews and research diary. During field work, the starting point was the documentary observation and the empirical study was conducted through semi-structured interviews followed by notes in the research diary. The survey analysis was made in light of Institutional Analysis in the perspective of Savoye and the report was of cartographic inspiration. The study showed that the city of Vitoria is avant-garde in the process of organizing the Pharmaceutical Assistance in Brazil, complying with the minimum guidelines of that process, keeping updated its List of Essential Medicines and, while in most Brazilian cities, the pharmacist is a professional of municipal and even regional reference, in our context this professional is present in almost all services - primary health care, referral centers, emergency rooms, health surveillance – which involve all the municipal healthcare system. As a challenge, the study pointed out that, despite the level of excellence in attending the guidelines of National Drug Policy, the physical structure of pharmacies that compose the services and continuing education, compromise the full implementation of pharmaceutical care to the users of the Unified Health System. For the administrators and managers interviewed, except for the political changes during the study period, a convergence of favorable factors - organizational restructuring of the Municipal Health Office in Vitória, local standards in Pharmaceutical Assistance, computerization of services and the acquisition of new pharmaceuticals through contests over the perio - made possible the anticipation of the national model of reorientation of Pharmaceutical Assistance so that when the Implementation of the National Drug Policy in 1998 came up, the municipality had already complied four o the eight guidelines contained in that policy. Therefore, the process of institutionalization of Pharmaceutical Care in Vitoria was ruled by a collective effort of the involved ones - administrators and managers – who have incorporated in their daily work, a wish to turn it a reality.
- ItemAdaptação transcultural do instrumento "tool to estimate patient's costs" para estimar os custos do tratamento da tuberculose aos pacientes em municípios prioritários do Brasil(Universidade Federal do Espírito Santo, 2016-10-08) Nunes, Gabriela Ferreira; Maciel, Ethel Leonor Nóia; 1º membro da bancaTuberculosis is historically associated with poverty, being a serious public health problem with deep social roots. The disease contributes to the patient's impoverishment and their families, generating costs due to treatment of the disease, which are classified into direct and indirect. This study aims to adapt culturally the Tool to Estimate Patients Costs instrument for the Portuguese of Brazil, used to estimate TB treatment costs for patients in three priority counties in Brazil. Study type Cost of illness by using a sample of 77 patients with closed questionnaire and pre-established by World Health Organization. The instrument showed a Cronbach's alpha above 0.71 in its translated version, and some modifications were necessary to create an instrument adapted to Brazil in a synthesized version, due to questions that do not apply to the reality of the tuberculosis treatment in Brazil. It is recommended to use the synthetic version of the questionnaire as a standard adapted to Brazil, so that this data can be part of the national health statistics according to their particularities in order to minimize the economic impact and the impoverishment caused by the disease to the patients and their families.
- ItemAdesão medicamentosa e fatores associados em idosos de um ambulatório filantrópico do Espírito Santo(Universidade Federal do Espírito Santo, 2013-05-08) Arruda, Daiane Campos Juvêncio de; Morelato, Renato Lírio; Oliveira, Elizabete Regina Araújo de; 1º membro da bancaThe incorrect use of medications, underuse, the irrational use or not use the total prescribed medication, are forms of non-adherence to medication. The prevalence ranges from 40 to 60% in the general population and can not be seen as an isolated phenomenon. Should evaluate the individual characteristics, the sociodemographic, health conditions and the use of drugs of the subject. The lifestyle and sociodemographic conditions may be associated with medication nonadherence due regard to the health of the individual. The aim of this study was to estimate the prevalence and factors associated with medication non-adherence in the elderly. Descriptive study, analytical, through interviews using a script prepared, organized and divided into blocks according to sociodemographic variables, health status, lifestyle and medications taken. To verify the non pharmacological treatment we used the instrument Adherence Measure Therapeutics. The sample consisted of 270 individuals aged 60 years or older with cardiovascular disease and who underwent outpatient treatment at the Hospital Santa Casa de Misericordia de Vitoria. Descriptive statistics, tests of association by chisquare, prevalence ratios and 95% confidence. Multivariate analysis was performed for multiple Poisson regression. Of the 263 respondents selected for the study, 26.7% reported not adhering to drug treatment. The findings of this research indicate that the lack of employment of the occupation earlier, the presence of cognitive decline and poor diet are associated with medication non-adherence in this population. Thus, the mental state, behavior of healthy living and better socioeconomic conditions may be factors in medication adherence in elderly outpatients.
- ItemAnálise espacial dos casos notificados de sífilis em gestantes e sífilis congênita no Estado do Espírito Santo(Universidade Federal do Espírito Santo, 2017-05-03) Soares, Karllian Kerlen Simonelli; Prado, Thiago do Nascimento; Miranda, Angélica Espinosa; 1º membro da bancaThe syphilis is considered a public health problem, because in addition to increasing incidence rates it also has deleterious effects on the fetus when it occurs in pregnancy women. Perform a spatial analyses is great benefit, because is ideal tool for evaluate the distribution and identify areas with higher concentration of cases. This study has objective of the analyze the spatial distribution of the cases in syphilis in pregnancy and congenital syphilis in the state of Espírito Santo in the period from 2011 to 2015. It’s about in ecological study with spatial analyses of cases of syphilis notified in notification of injury information system using the software STATA 13 and Terra View 4.2.0. In study were included all reported cases syphilis in pregnancy and congenital syphilis. On both maps is possible to observe a main cluster in the metropolitan area and north of the state. Numerous municipalities with high rate the detection of the syphilis in pregnant woman in some municipalities and may indicate that carrying out the notification of cases. However many municipalities did not present any rate of detection of syphilis in pregnant women suggesting underreporting the cases. In the many municipalities there is a low detection rate of syphilis in pregnant woman but high rates of congenital syphilis evidencing underreporting. The study evidenced priority areas in acting and municipalities in the high risk of disease. It is necessary also training and education in health of this population and health professionals.
- ItemAssociação entre consumo de antioxidantes e risco de doenças coronarianas em participantes do ELSA-Brasil(Universidade Federal do Espírito Santo, 2014-01-01) Teixeira, Marina Galvão; Mill, José Geraldo; Molina, Maria del Carmem Bisi; 1º membro da bancaLow plasma antioxidant concentrations are associated with endothelial dysfunction, the first step towards atherosclerosis. Some antioxidant compounds, presents in some food kind, are capable to minimize those disturbances and are associated with the reduction of cardiovascular risk. We aimed to identify the association between the intake of antioxidant nutrients and the risk of acute coronary disease in ten years in the participants of the Longitudinal Study of Adult Health – ELSA Brazil. 14,353 participants aged of 35-74 years, which are employees of six public universities, were studied. The estimated intake of antioxidant nutrients (Vitamins A, E and C, zinc and selenium) was obtained through a food frequency questionnaire (FFQ), adjusted for total energy and categorized into quintiles. The coronary risk (CR) was calculated using the following variables: sex, age, systolic blood pressure, smoking, antihypertensive therapy, HDL-cholesterol and total cholesterol. Participants were classified into risk categories (low, intermediate and high). Analysis of simple and multivariate regressions were performed to assess the relationship between the consumption of antioxidants, CR and the influence of other variables (education and BMI). It was observed that 71 % participants were at low risk, 7 % in intermediate risk and 22 % at high CR. Female participants, those with higher income, education and age, had higher estimated intake of antioxidants. Positive and statistically significant associations between intake of antioxidants and high CR were observed. Individuals with intermediate risk had the lowest intake of antioxidants. Education level was positively associated with consumption of antioxidants (p < 0.001) and CR (p < 0.001). After adjustment for body mass index and education, low antioxidant intake remained associated with the intermediate CR (OR = 1.74. 95 % CI 1.37 to 2.21). Positive relationship between CR and intake of antioxidant nutrients may be associated with a change in eating habits after a cardiovascular event or due to some recent medical diagnosis.

- ItemFatores associados à tuberculose em população de diabéticos : um estudo caso-controle(Universidade Federal do Espírito Santo, 2016-03-30) Leal, Marcelle Lemos; Cade, Nagela Valadão; 1º membro da bancaIndividually, diabetes mellitus (DM) and tuberculosis (TB) have an important impact on public health worldwide. There is evidence that people with DM have a three times higher risk of catching TB than the general population’s susceptibility to infections. This study investigates the factors associated to developing tuberculosis in patients with diabetes mellitus being treated at health units in Vitória/Espírito Santo. It is a case-control study, with the case group comprising diabetic patients treated at the 30 units in the municipality and notified on the National Disease Notification System (SINAN), with a diagnosis of tuberculosis during the period 2007 to 2013, and the control group comprising people with diabetes. Data was used from SINAN, the Health Information System at the Department of Health in Vitória and Central Municipal Laboratory. An interview held with a semi-structured script was applied to 45 cases and 92 controls, considering social, economic and demographic variables, lifestyle habits and clinical characteristics of diabetes. Bi-varied and multivaried analyses were performed by means of logistic regression with significant variables (p<0.05). The case group presents worse lifestyle habits compared with the control group, such as higher regular consumption of alcohol (p=0.008), compulsive consumption of alcohol (p<0.001), smokes more (p=0.051), in addition to worse biochemical parameters, such as fasting blood sugar (p<0.001) and higher glycosylated haemoglobin (p=0.027). Higher regular consumption of alcohol (OR 5.78; IC 1.726-19.359), previous contact with people with tuberculosis (OR 3.773; IC 1.161-12.258) and fasting blood sugar (OR 1.012; IC 1.001-1.024) were associated to tuberculosis in those with diabetes. As a result, this study reiterates the need for integration between the tuberculosis and diabetes mellitus control programmes for screening, confirming diagnosis and the joint handling of these diseases.

Results: In teaching, homeopathy and phytotherapy were optional subjects in undergraduate medicine, pharmacy and nutrition courses; in nursing, there was a mandatory subject that addresses the subject of PICS; in the multi-professional residency postgraduate course there is a PICS discipline on the transversal axis, which is mandatory; In the master's and doctorate programs in public health, master's degree in nursing and pharmaceutical sciences, there is also a record of related subjects and content. In research, there are two research groups, and the record of 174 scientific publications; in extension, 19 activities. Topics generally stand out in PICS, and specifically related to medicinal plants/phytotherapy in teaching (60%) and research (20.1%); and meditation (26.3%) in extension. Conclusions: the situational diagnosis highlighted the presence of PICS in undergraduate and postgraduate education, the existence of research groups and developed research and extension activities in the university's CCS
- ItemPrograma Mais Médicos : limites e possibilidades para mudanças nos processos de trabalho e na produção do cuidado na estratégia saúde da família(Universidade Federal do Espírito Santo, 2017-09-25) Boiteux, Pablo de Almeida; Sarti, Thiago Dias; Lima, Rita de Cássia Duarte; 1º membro da bancaThis study analyzes the effects of the insertion of a Cuban doctor of the Mais Médicos (More Doctors) Program (PMM) into the work processes of a Family Health Strategy (FHS) team, as well as discusses its limits and possibilities for the production of changes towards the comprehensiveness of health care. This is a qualitative, descriptive-exploratory research, of the case study type, carried out in a city of the state of Espírito Santo, Brazil, having as unit of analysis the team in which the referred doctor works. Also participating in the study were a nurse, a nursing technician and six community health agents. Empirical data production techniques were used: participant observation of the team's assistance activities; in-depth interviews with the Cuban physician and the nurse, and a focus group, in which the nursing technique and the community health agents participated. Data analysis was based on theories of the work process and intersubjective production of health care from the perspective of comprehensiveness. The results indicate contributions in the expansion, diversification and continuity of care actions, as well as in the strengthening of the links between team and users, especially in the doctor-user relationship. Among the challenges, the persistence of practices segmented by professional categories, subordinate to the (bio)medical figure and knowledge, with limited interprofessional and team-community interactions in the construction of common projects of care. It is hoped that this study will stimulate future debates and research on PMM in the field of Collective Health and contribute to the exchange of knowledge related to the work processes in Primary Health Care (PHC) and the Unified Health System (Sistema Único de Saúde - SUS), bringing them closer the needs of the Brazilian population and the workers themselves, and offering subsidies for the improvement of health care in Brazil.
